Maybe it's her/his twins?
I read it in the tankoubon that he/her had twin and the other twin is a sister.

But still... I'm also starting to think that hoshino it's a girl.

I've been pretty sure for a long time that Hoshino is female, but this more or less confirmed it.

Besides, it can't really be anyone else, because that image panel is where the author is supposed to put a picture of themselves--drawn or otherwise. So it'd be pretty silly to put anyone else there.

Well I had read the other post that in the fanbook that stated hoshino is a she ^^a
Well... I'm the type that read the comic without thinking is it a she or a he who make it XD so I kinda late thinking of it :D
